
Our surgery is based in a Grade II listed building that has been converted and extended to provide an attractive environment from which to offer a high standard of healthcare.

Using Online Consultations allows us to quickly and safely decide on the best way to help you, your child, or an adult in your care. We will aim to respond to your query within 72 hours during our opening hours
PLEASE DO NOT SUBMIT MEDICAL REQUESTS AS AN ADMIN REQUEST. THEY WILL NOT BE DEALT WITH AND WILL BE RETURNED TO YOU TO RESUBMIT AS A MEDICAL REQUEST. THIS COULD RESULT IN DELAYS IN MEDICAL CARE. IF ADVISED TO CALL THE SURGERY OR ATTEND A+E OR CALL 999/111 WHILST COMPLETING THE QUESTIONNAIRE PLEASE DO NOT IGNORE THIS ADVICE. THIS ONLINE FACILITY IS NOT AN EMERGENCY/URGENT SERVICE.
IF THIS SERVICE IS ABUSED AGAIN WE WILL HAVE TO SWITCH IT OFF AGAIN PERMANENTLY.
PATIENT SAFETY IS PARAMOUNT.
It would be a shame to lose this service which improves patient convenience for more routine medical problems, because of a few people misusing it.
The number of medical requests is currently capped to maintain patient safety. We aim to increase the number of requests available each day over time in a controlled way to ensure this service remains safe to use. The "opening hours" of this service should also hopefully be extended as long as this remains safe.

Additional Features
24 Hour Automated Telephone System
Forest House Surgery has a new telephone system with some new features.
The new telephone system enables patients to make pre-bookable appointments with a Doctor, up to a month in advance, and also check, amend or cancel any other appointments at the surgery. This will give patients greater control over booking appointments.
The line is now open 24 hours a day, 7 days a week, so you no longer have to wait for the surgery to open to book your appointment.
Please note this is for a pre-bookable appointment not for same day appointments. To book a same day appointment please speak to receptionist.
Online Services

Our patients can now use the internet to:
- Book and cancel appointments with a GP
- Request repeat prescriptions for any medication they are taking regularly
- Look at their coded medical record
For access to this facility please ask at reception for a username and password, Please be aware there is a form to fill in and we will need identification to register you, for example- Passport, Picture Driving Licence, Utility Bill/Bank Statement dated within 3 months (not mobile phone).
Disabled Facilities
We offer reserved car parking spaces for the disabled and wheelchair access to our building. A disabled patients' WC is provided and there is a door chime at the front entrance to alert staff to patients requiring help with entering the building. We also have available wheelchairs to transfer patients from the car park in to the surgery, please ask at reception if you require this service. There is also a hearing loop on our reception for the hard of hearing. We now offer parking facilities for mobility scooters.

Accessible Toilet and Baby Changing Facilities


Baby Friendly
Within the surgery we offer a baby changing area. We appreciate that you may require to feed your baby and we are a breast feeding friendly practice. If a nursing mother would like additional privacy please ask at reception. Feel free to bring prams and pushchairs in to the surgery.
Self-Testing Blood Pressure (BP) Machine
Within the main reception area we offer the ability for patients to use a self-testing BP machine. No appointment is necessary to use this machine. These results are handed in to reception and we ensure these are inputted to your record and viewed by a GP.
N.B. If you have been asked to make an appointment with the GP or Nurse for a BP check, you must still do so.

Self Check In Screens
In our main reception we have two Self Check In screens. This allows you to sign in without the need to queue at the reception area.
