TOTAL TRIAGING 22ND SEP

We would like to remind our patients that Total Triage will be implemented at Forest House Surgery from Monday, 22nd September. If you require an appointment for yourself or anyone else, you must submit a medical request form through the ‘’I have a health problem’’ option. These will be available to submit on Monday 22nd September from 8:00am. If you are unable to complete a form you can call the surgery or if you prefer you can visit the surgery.
You do not need the NHS app to submit a medical triage form, you can do this via our website selecting ‘’Contact us’’.
We understand that changes to the way appointments are accessed may cause some uncertainty. Please be assured that our staff are fully trained and available to support you through this transition.
